To qualify for the ITIL Expert, you must obtain at least 17 credits from the Foundation, Practitioner and Intermediate modules, and pass the Managing Across the Lifecycle (MALC) exam, earning a total of 22 credits.

ITIL Master Certificate – IT Service Management

One of our favorite credential sets (and for employers as well, judging by job board numbers) is the ITIL for IT Service Management credentials from Axelos. Axelos is a global provider of standards designed to drive best practices and quality throughout organizations. ITIL (Information Technology Infrastructure Library) joined the Axelos family in 2013.

Axelos manages ITIL credentialing requirements and updates, provides accreditation to Examination Institutes (EIs), and licenses organizations seeking to use ITIL. In addition to ITIL certifications, Axelos offers credentials for Prince2 2017 (which includes Foundation, Practitioner and Agile qualifications), Prince2 Agile, Resilia, MSP, MoP, M_o_R, P30, MoV, P3M3 and AgileSHIFT.

ITIL is a set of well-defined and well-respected best practices that specifically target the area of IT service management. There are more than 2 million ITIL-certified practitioners worldwide. ITIL is perhaps the most widely known and globally adopted set of best practices and management tools for IT service management and support.

ITIL Foundation (2011): Complete course and 2 practice exams

Axelos maintains a robust ITIL certification portfolio consisting of five ITIL credentials:

  • ITIL Foundation: An entry-level credential that validates general ITIL knowledge, including terminology, concepts, elements, services lifecycle and ITIL processes
  • ITIL Practitioner: A steppingstone credential for the Intermediate credential that tests a candidate’s ability to use ITIL principles within their business organization

The ITIL Master is the pinnacle ITIL certification, requiring experience, dedication, and a thorough understanding of ITIL principles, practices, and techniques. To gain the ITIL Master designation, candidates must have at least five years of managerial, advisory or other leadership experience in the field of IT service management. They must also possess the ITIL Expert certification. Once the skill and certification requirements are met, the real certification work begins.

Upon completing the prerequisites, candidates must register with PeopleCert, the sole approved Axelos Examination Institute, and submit an application. Next, candidates prepare and submit a proposal for a business improvement to implement within their organization. The proposal submission is followed by a “work package,” which documents a real-world project that encompasses multiple ITIL areas.

The work package (1) validates how the candidate applied ITIL principles, practices, and techniques to the project; and (2) documents the effectiveness of the solution and the ultimate benefit the business received as a result of the ITIL solution. Finally, candidates must pass an interview with an assessment panel where they defend their solution.

TOGAF 9

A leader in enterprise architecture, The Open Group’s standards and certifications are globally recognized. The TOGAF (The Open Group Architecture Framework) standard for enterprise architecture is popular among leading enterprise-level organizations. Currently, TOGAF is the development and architecture framework of choice for more than 80% of global enterprises.

TOGAF’s popularity reflects that the framework standard is specifically geared to all aspects of enterprise-level IT architectures, with an emphasis on building efficiency within an organization. The scope of the standard’s approach covers everything from design and planning stages to implementation, maintenance, and governance.

The Open Group offers several enterprise architect credentials, including TOGAF, Open CA, ArchiMate, IT4IT and the foundational Certified Technical Specialist (Open CTS).

The Open Group reports that there are more than 75,000 TOGAF-certified enterprise architects. At present, there are two TOGAF credentials: the TOGAF 9 Foundation (Level 1) and TOGAF 9 Certified (Level 2). (The TOGAF framework is currently based on version 9.2, although the credential name still reflects version 9.)

The TOGAF 9 Foundation, or Level 1, credential targets architects who demonstrate an understanding of TOGAF principles and standards. A single exam is required to earn the Level 1 designation. The Level 1 exam focuses on TOGAF-related concepts such as TOGAF reference models, terminology, core concepts, standards, ADM, architectural governance and enterprise architecture. The Level 1 credential serves as a steppingstone to the more advanced TOGAF Level 2 certification.

The TOGAF 9 Certified, or Level 2, credential incorporates all requirements for Level 1. Level 2 TOGAF architects possess in-depth knowledge of TOGAF standards and principles and can apply them to organizational goals and enterprise-level infrastructure. To earn this designation, candidates must first earn the Level 1 credential and pass the Level 2 exam. The Level 2 exam covers TOGAF concepts such as ADM phases, governance, content framework, building blocks, stakeholder management, metamodels, TOGAF techniques, reference models and ADM iterations.

Candidates wanting a fast track to Level 2 certification may take a combination exam, which covers requirements for both Level 1 and 2. Training is not mandatory for either credential but is highly recommended. Training classes run 2-5 days, depending on the provider and whether you’re taking the combined or single-level course. The Open Group maintains a list of approved training providers and a schedule of current training opportunities on the certification webpage.

Zachman Certified – Enterprise Architect

Founded in 1990, Zachman International promotes education and research for enterprise architecture and the Zachman Framework. Rather than being a traditional process or methodology, the Zachman Framework is more accurately referred to as an “ontology.” Ontologies differ from a traditional methodology or process in that, rather than focusing on the process or implementation, they focus on the properties, types and interrelationships of entities that exist within a particular domain. The Zachman Framework ontology focuses on the structure, or definition, of the object and the enterprise. Developed by John Zachman, this framework sets a standard for enterprise architecture ontology.

Zachman International currently offers four enterprise architect credentials:

  • Enterprise Architect Associate Certification (Level 1): Candidates must attend a four-day modeling workshop and pass a single exam. The workshop covers key concepts relating to enterprise architecture and the Zachman Framework, case studies, engineering vs. primitive models and manufacturing vs. composite models, and hands-on experience building Framework models. The workshop fee ($3,499) includes the exam and certification fees for Level 1 and Level 2.
  • Enterprise Architect Practitioner Certification (Level 2): Architects must submit case studies of primitive and composite models that address specified management issues. Case studies must pass a referee review.
  • Enterprise Architect Professional Certification (Level 3): Candidates must complete a case study demonstrating primitive (architectural) and composite (implementation) models and complete a referee review. Level 3 credential holders may advertise themselves as “Zachman consultants.”
  • Enterprise Architect Educator Certification (Level 4): Designed for educators, this credential prepares candidates to develop and teach the Zachman Framework. To earn this credential, candidates should go through all educational materials related to the Zachman Framework, develop their own curricula and course materials, and present those materials for review and approval. While this is not required, Zachman recommends that Level 4 candidates obtain the Level 3 Professional designation.

Zachman credentials are valid for three years. To maintain these credentials, candidates must earn continuing education credits (referred to as EADUs). The total number of EADUs required varies by certification level.

Beyond the top 5: More enterprise architect certifications

The Red Hat Certified Architect (RHCA) is a great credential, especially for professionals working with Red Hat Enterprise Linux.

The Project Management Professional (PMP) certification from PMI continues to appear in many enterprise architect job descriptions. Although the PMP is not an enterprise architect certification per se, many employers look for this particular combination of skills.

Outside of our top five vendor-neutral enterprise architect certifications (which focus on more general, heterogeneous views of IT systems and solutions), there are plenty of architect-level certifications from a broad range of vendors and sponsors, most of which are vendor-specific.

These architect credentials typically represent pinnacle certifications within the programs to which they belong, functioning as high-value capstones to those programs in many cases. The group of individuals who attain such credentials is often quite small but comes with tight sponsor relationships, high levels of sponsor support and information delivery, and stratospheric salaries and professional kudos.

Often, such certifications provide deliberately difficult and challenging targets for a small, highly select group of IT professionals. Earning one or more of these certifications is generally the culmination of a decade or more of professional growth, high levels of effort, and considerable expense. No wonder, then, that architect certifications are highly regarded by IT pros and highly valued by their employers.

Choosing the right IT architect credential

Enterprise architect credentials will often be dictated by choices that your employer (or industry sector, in the case of government or DoD-related work environments) have already made independent of your own efforts. Likewise, most of the vendor-specific architecture credentials make sense based on what’s deployed in your work environment or in a job you’d like to occupy.

Though there are lots of potential choices IT pros could make, the actual number they can or should make will be influenced by their circumstances.

A Colwood couple considered themselves lucky amid a family-doctor shortage to become patients of a nurse practitioner three years ago, but are shocked to find their ­insurance company won’t cover an orthotics bill because it wasn’t prescribed by a “physician.”

Robert and Andrea Brown, both in their 70s, were without a primary-care clinician until Andrea Brown had to visit the Urgent and Primary Care Centre in Goldstream and was connected with nurse practitioner Sarah Adair.

The couple have been reimbursed over the years for their prescription drug costs through insurer Canada Life, but when it came to a $500 bill for Andrea’s custom-made orthotics to alleviate hip and knee pain, the insurance company said that under Brown’s plan, that type of prescription had to be written by a “physician, podiatrist or chiropodist.”

“They say nurse practitioners are not covered,” said Andrea Brown, 75. “I think they should be covered.”

There are more than 800 nurse practitioners in B.C., about half of whom are working in family practice, according to the Nurse and Nurse Practitioners of B.C.

Robert Brown, 73, has an extended-health-care plan that covers his wife, from the University of Waterloo, where he was a faculty member. He can’t fathom why Canada Life — owned by Power Corporation of Canada — would cover far more serious medication but not orthotics.

“It’s one of the things that boggles the mind because [our nurse practitioner] writes my heart-medication scrips, and I got all my reimbursements for those,” he said. “And yet for this one, because of the legal wording of the contract between my previous employer and the insurance entity, they’re claiming they’re not going to pay because the script has to be written by a medical doctor.”

Robert Brown said he appealed, noting that “like a million other folks in B.C., I did not have a medical doctor and probably would never have one,” but Canada Life rejected the appeal.

“Under the terms of your plan, custom made orthotics are covered when prescribed by a physician, podiatrist or chiropodist,” says the June 22 letter from Canada Life. “As a nurse practitioner is not eligible to prescribe custom made orthotics under your plan, we must maintain our decision to decline your claim.”

A Canada Life spokesperson could not be reached for comment.

The Health Ministry said Tuesday that it was “discouraged” to hear that a prescription from a nurse practitioner had been rejected. “We encourage all insurance companies in the province to recognize nurse practitioners’ ability to prescribe, in order to connect more people with the care and services they need,” the ministry said in a statement.

Nurse practitioners are registered nurses educated at a masters level and have advanced training. Paid on salary, they can work on their own or with physicians and other health professionals to provide care that includes diagnosing and treating physical and mental illnesses, ordering and interpreting tests, prescribing medications and performing medical procedures. A nurse practitioner cannot, however, certify someone under the Mental Health Act as a physician can.

The Nurses (Registered) and Nurse Practitioners Regulation and B.C. College of Nurses and Midwives establish the nurse practitioners’ scope of practice.

Michael Sandler, CEO of Nurses and Nurse Practitioners of B.C., said in a phone interview this week that he doesn’t understand why the insurance company wouldn’t accept the signature of a nurse practitioner for an orthotic prescription, adding it’s not the first time it has happened.

“Some insurance providers — for reasons that remain with those insurance providers — do not accept the qualified signature of a nurse practitioner as it relates to some paperwork pieces,” said Sandler, noting that it’s long been enshrined that nurse practitioners can refer to specialists.

The issue ends up being a barrier for patients, he said. “We would obviously like … certain private insurance companies to play by the rules, but ultimately, that rests with them as an organization.”

Sandler said the Nurses and Nurse Practitioners of B.C. wants to expand the scope of practice for nurse practitioners, which might help alleviate situations like the one the Browns are facing.

“That’s an inefficiency within the system that we advocate strongly to try and ameliorate based on the fact that nursing offers a multitude of solutions to some of the health-care delivery challenges the system is currently facing,” said Sandler.

UBC started offering a nurse practitioner program in 2003. Four years ago, Health Minister Adrian Dix announced $115 million to create 200 more spaces for nurse practitioners over three years.

BUFFALO, N.Y. (WKBW) — The demand for nurse practitioners continues to increase. The U.S. Bureau of Labor Statistics said nearly 113,000 jobs are expected to be added to the economy by 2031.

Enrollment in the DNP Program, the nurse practitioner program, at the University at Buffalo's School of Nursing has doubled since 2018.

"It takes them to a new level not just knowledge wise but fulfillment wise. They know this stuff. It's just formalizing it," Pamela Paplham, the Associate Dean of Clinical Programs at the University at Buffalo School of Nursing, said.

Some students have come back to school after working in the medical field.

"I worked on the floor for about seven years. Then I worked in management for the past 10 years. I've done a lot of different things. I've been in every area of the hospital at one point or another," Emily Vezina, a first year student in the nurse practitioner program, said.

Other students got their undergraduate nursing degree, passed their exams to become a registered nurse, and went straight into the nurse practitioner program.

"I didn't want to stop. I didn't want to take a break and then feel comfortable or complacent. I was just like I want to keep going. This was my goal," Jasmine Silvagnoli, a third year student in the nurse practitioner program, said.

Annette Wysocki, the Dean of the University of Buffalo School of Nursing, said the number of newly licensed nurse practitioners in New York has grown from just over 2,000 in 2017 to more than 3,500 in 2021.

There are 27,000 nurse practitioners licensed in New York as of January 1, 2022.

"I think the perspective of the nurse, especially because all of us have worked at the bedside, we know what it's like to take care of patients twelve hours at a time," Vezina said.

Wysocki said nurse practitioners are expected to fill the primary care gap going forward. She said they already provide about 30% of primary care in the United States.

Nurse practitioners can specialize in any field of medicine. New York State does not require nurse practitioners to work under a physician, so they can open their own practice. Silvagnoli plans to do just that by opening her own med-spa when she graduates.

"I can do that as a nurse practitioner and not get the MD, be in school for four years, then do residency. God knows how much debt I would be in after that just to do the same exact thing," Silvagnoli said.

The advanced skills that a nurse practitioner brings to the U.S. healthcare system are so in demand that, as of 2022, 27 states allow nurse practitioners to practice independently, according to the American Association of Nurse Practitioners (AANP). This means NPs can run their own healthcare practice without the need for a collaborating physician in those states. That autonomy helps many communities, especially in rural areas, have access to convenient, affordable healthcare.

Who Are Nurse Practitioners?

Typically, they'll need to earn a Master of Science in Nursing (MSN) - Family Nurse Practitioner (FNP) with a specific population focus, such as pediatrics, family medicine or gerontology. They must also pass a national certification exam before applying for state licensure.

While nurse practitioners can work in virtually any area of medicine or healthcare, their role “often emphasizes disease prevention and health management in caring for their patients,” said Dr. Nicholas S. Carte, AGPCNP-C, APRN, graduate nursing faculty lead at Southern New Hampshire University (SNHU).*

What Does a Nurse Practitioner Do?

In addition to providing clinical care, nurse practitioners focus on communication and patient education. Incorporating these practices helps put patients at ease and increases the likelihood that patients will more "willingly participate" in their own healthcare, said Dr. Jequie Dixon, APRN, AGACNP-BC, clinical coordinator of MSN programs at SNHU.**

Nurse practitioners can provide quality healthcare in virtually any medical setting. They may:

  • Address health concerns
  • Assess the health status of patients
  • Collaborate with an interdisciplinary team
  • Diagnose acute and chronic diseases
  • Order and interpret labs and diagnostic imaging
  • Prescribe pharmacological and non-pharmacological treatments

Becoming a nurse practitioner enables nurses to advance their careers, building upon a solid nursing practice foundation to diversify specialty skill sets. Professional growth was critical throughout the COVID-19 pandemic. "The nurse practitioners stood up and showed their value by providing high-level care to patients in need during the overwhelming healthcare crisis of the COVID-19 pandemic," Dixon said.

Many people also assume that nursing is the same in every setting. “Being in the military, I was able to work in many states,” said Dixon. That wide view of the nursing profession helped her see the differences and opportunities for nurses throughout the country. Dixon now puts her skills and experience to work with program development and management for SNHU. She is also teaching and practicing as an NP hospitalist.

What is an FNP?

A family nurse practitioner, or FNP, is an RN with additional education and training in primary care settings to treat patients across the lifespan. They may earn an MSN - Family Nurse Practitioner degree or a Doctor of Nursing Practice (DNP).

While FNPs are healthcare providers for patients of any age in the primary care setting, nurse practitioners can specialize in specific populations, such as adult-gerontology, pediatrics, women, neonatal and psychiatric/mental health. Within gerontology and pediatrics, nurse practitioners may train, become certified and then practice in acute or primary care settings, whereas a family nurse practitioner “has the opportunity to practice in various clinical settings,” said Carte.

Nurses with the FNP specialty work similarly in scope to a family practice physician who treats patients of all ages. They address all manner of health concerns, administer physicals and deliver patient education to help patients achieve health and wellness. While a family nurse practitioner does treat illness and disease, their focus is often on helping their patients get and stay healthy.

One of the many benefits of being a family nurse practitioner is the flexibility to work in such broad clinical settings. FNPs are the “jack of all trades” of nurse practitioners, according to Dixon. “They can work in pediatric offices, nursing homes, or home healthcare. They can really do a lot,” she said. Dixon specializes in acute care as a hospitalist nurse practitioner.

What is the Difference Between a Nurse Practitioner and a Physician?

The main difference between a nurse practitioner and a physician is the type of training. While both roles play a necessary part in patient education and communication, physicians are trained in the medical model, whereas nurses are trained using the nursing model.

The notable differences between the two are:

  • Different licensing requirements by their respective state licensing boards
  • The amount of time spent pursuing a degree required to practice

Physicians are licensed to practice independently in every state and can work without supervision or oversight from another practitioner. Nurse practitioners can practice independently in many states and work under a collaborative agreement with physician oversight in other states.

That list of states where nurse practitioners can practice independently continues to grow. "This is a wonderful trend," said Dixon. "Getting providers to set up practice in smaller towns is difficult, so having more nurse practitioners there allows for even more impact to these smaller communities."

What Skills Do Nurse Practitioners Need?

While excellent communication and critical thinking skills are at the top of the list of important skills for the nursing profession, good nurse practitioners also have the following skills:

  • Ability to accurately collect patient health history and conduct physical exams
  • Coordinate care from hospital admission to discharge
  • In outpatient settings, provide primary care, including managing referrals to specialty providers
  • Interpret labs and other resources and prescribe medication
  • Strong active listening, organizational and time management skills
  • Use current technology in healthcare and stay up to date with any changes

Skilled nurse practitioners are “proactive in therapeutic health and develop health promotion plans,” Carte said. In addition, they must “understand and recommend vaccine schedules, have the ability to counsel and educate patients and document cases based on accepted standards of care.”

The list doesn’t end there. Nurse practitioners should also “demonstrate sound risk management skills, understand healthcare policy and procedures and understand the legal and ethical aspects of being a nurse practitioner, which can vary by state,” said Carte.

What are Common Misconceptions About Nurse Practitioners?

While nursing is a respected profession, it’s also a diverse one. With the different nursing specialties and certifications available, it’s not surprising that the public isn’t always sure about the finer points of the role.

Some common misconceptions about nurse practitioners include:

  • That they need to work during the times a physician is present
  • That they can’t work independently
  • That they can’t see their own patients and have their own caseloads
  • That they are not able to prescribe medications
  • That they cannot diagnose a patient with a health concern

None of these are true, especially if a nurse practitioner is in a state where they can practice to the fullest extent of their license. Nurse practitioners can put their training, education and expertise to use seeing patients, managing their own caseloads, prescribing medications and diagnosing patients.

More than that, they can work collaboratively with patients and the entire healthcare team to create a treatment plan. It’s important for their patients to feel comfortable with their treatment, so they stick with it and find success. That often involves an explanation of the details in a way the patients can understand.

“A lot of people think that nurses just implement tasks,” Dixon said. But a skilled nurse “thinks through everything that the patient is going through.” Nurse practitioners consider the whole patient, including financial aspects of receiving medical care. “I may write a prescription for a patient but find out that the medication is a financial burden for the patient,” she said. “So, I would have that conversation with the patient to best help them and perhaps find an alternative treatment or medication. I work to support patients in every aspect of managing their healthcare.”

Where Are Nurse Practitioners Needed?

There’s no doubt that nurses are a critical component of our healthcare system. Becoming a nurse practitioner allows a registered nurse an even broader reach to help patients achieve wellness and remain healthy. Because more and more physicians are focusing on specialty practices in highly populated areas like cities, the potential to serve the public as a nurse practitioner is greater than ever.

Nurse practitioners can serve as healthcare providers in settings such as:

  • Home healthcare: Many patients are not sick enough to require hospitalization but need assistance with their healthcare needs, perhaps post-surgery or post-illness. Home healthcare may also include palliative care. Pain management, medication education and overall healthcare support in one’s home are areas of focus for many nurse practitioners.
  • In-patient hospitals: Nurse practitioner hospitalists focus on patients who are admitted to the hospital. They may further specialize as gerontological hospitalists, pediatric hospitalists or hospitalists in other specialty areas. Patient communication and education are key here as well.
  • Outpatient urgent care settings: Nurse practitioners are ready to help treat patients with acute health or injury issues, and they may order and interpret test results or prescribe medication.
  • Private practice: In this broad clinical setting, nurse practitioners may see patients of all ages for various health issues. In some states, nurse practitioners will work under the oversight of a physician, but in many states, they can legally work as independent or autonomous practitioners.
  • Skilled nursing facilities (SNF): SNFs offer an opportunity for nurse practitioners to serve as providers on the treatment team.

Rural areas are also highly in need of additional healthcare options and providers. “As the healthcare system continues to see a decrease in primary care providers,” Carte said, “nurse practitioners can support the need for additional primary care providers in rural areas.”

The California Board of Registered Nursing released proposed regulations setting forth requirements for the new categories of nurse practitioners created in 2020 by Assembly Bill 890, pushing nurse practitioners closer to an independent scope of practice.

In a notice dated September 6, 2022, the California Board of Registered Nursing (BRN) released a Notice of Proposed Action to Article 8, Division 14 of Title 16 of the California Code of Regulations (CCR). The proposed regulations would add sections 1482.3 and 1482.4 of the CCR. 

These regulations have been long-awaited following the passage of Assembly Bill 890 (AB 890) in September 2020. As we previously reported in our Health Care Counsel Blog, AB 890 amended the Nursing Practice Act, broadening the nurse practitioner (NP) scope of practice and allowing for greater NP independence. Specifically, AB 890 created two new categories of NPs under California Business and Professions Code sections 2837.103 and 2837.104. The new categories of NPs — referred to as Section 103 NPs and Section 104 NPs — can function independently within a defined scope of practice and without standardized procedures. However, no NP could qualify for these new categories until the BRN issued implementing regulations required by AB 890.

The Proposed Transition-to-Practice Standards

The BRN’s proposed regulations address several AB 890 mandates, including the application process to become a 103 NP or 104 NP, minimum qualification requirements, and certain protocol and notice requirements. 

However, the most anticipated provision in the proposed regulation is the “transition to practice” definition.  

AB 890 requires NPs to complete a “transition to practice” to qualify for the expanded scope of practice as a 103 NP or 104 NP. “Transition to practice” is not defined in statute; rather, the BRN was tasked with defining in regulation minimum transition-to-practice standards. Under the proposed regulations, the “transition to practice” requirement would consist of a minimum of “4600 hours or three full-time equivalent years of clinical practice experience and mentorship” that meets all of the following requirements:

  • Completed in California;

  • Completed within five years prior to the date the NP applies for certification as a 103 or 104 NP;

  • Completed after certification by the BRN as an NP; and  

  • Completed in direct patient care in the category in which the NP seeks certification (these categories are defined in section 1481(a) of the existing regulations).

Additionally, to qualify as a 104 NP, the proposed regulations include an additional requirement that the NP has practiced as a 103 NP for at least three full-time equivalent years or 4600 hours in direct patient care.

Because the regulations are not yet final, no NP has been able to meet the qualifications for the expanded scope of practice. Pending their adoption, the release of proposed regulations defining the transition-to-practice standards brings NPs closer to an independent scope of practice in California. 

The BRN is accepting written comments to the proposed regulations, due by November 1, 2022, at 5:00 PM.
