Potential layoffs ahead

Oracle’s accurate cost-reducing considerations to save the company as much as $1 billion could possibly result in putting thousands of jobs on the chopping block as early as August, according to a report published Friday by The Information.

An Oracle spokesperson with knowledge of the situation said that the layoffs could “disproportionately impact” workers in the U.S. and Europe and would include marketing for software applications in customer service and e-commerce employees, the report said.

Oracle has not responded to CRN US’s request for comment on the possible layoffs.

The company last made substatial cuts in 2019 as part of a restructuring. Oracle, at the time, revealed it was cutting hundreds of jobs across the U.S. as it was looking to Excellerate its financials in what appeared to be part of a much larger restructuring of its 143,000-employee workforce.

We’ve now reached a significant milestone in that work: we’ve changed the default storage location of US user data,” TikTok’s blog said. “Today, 100 percent of US user traffic is being routed to Oracle Cloud Infrastructure. We still use our U.S. and Singapore data centres for backup, but as we continue our work we expect to delete US users’ private data from our own data centres and fully pivot to Oracle cloud servers located in the U.S.”

In a letter to Congress last month, TikTok also said Oracle Cloud will help it train machine learning models to Excellerate TikTok’s personalized video recommendations.

Since then, progress has been slow, but the industry now has everything it needs to close the feedback loop between science, evidence, and care, said speakers at a accurate Conference Forum webinar.

The key, according to Ryan Moog, former senior director of real-world data and clinical research at Oracle Cerner; Michael Ibara, chief data officer at Elligo; and Andy Stankus, general manager of real-world evidence at Cerner Eviza, formerly Kantar Health, was collecting and connecting the right data, and supporting sites to get involved.

More than a technology problem

We have known the potential of real-world data (RWD), captured by EHRs, for decades, said Ibara, highlighting a quote from the late 1990s paper, The Evolution in Healthcare Records in the Era of the Internet.

“What we need to move forward is an integrated clinical care programme… where we are continuing to learn and evolve, and to understand the best thing for patients, as well as for clinicians, clinical workflows, and communities.”

It pointed to the “many advantages” of the EHR when carrying out clinical research, such as eliminating the manual task of extracting data from charts and “making research data collection a by-product of routine clinical record keeping”.

Said Ibara: “If you compare the progress your phone has made since 1998, and the progress we have made in using healthcare data for clinical research, there’s a dramatic difference. If it were just a technology problem, we would have solved it by now. There are other factors involved.”

There are a number of things industry needs to do to “make this innovation stick”, and the first is changing the way it approaches study recruitment.

“In today’s clinical research areas, with the complexity of regulations, clinical trials, and the types of drugs we are trying to bring to market, we need a lot of patients,” he said. “But with our traditional methods, every site is their own fisherman: they set out their pole and they wait for the fish to come by.”

It’s an approach that makes enrolment a lengthy “trickle down” process that often contributes to delayed study initiation or even study discontinuation.

But access to large-scale EHR data can streamline the whole process by allowing researchers to quickly locate and identify patients with the right inclusion criteria, and then enrol them all at once.

There are barriers to this approach, not least the accurate “data land grab” that means no one vendor or organisation has access to all the patients needed for a clinical trial. This is particularly true when looking at rare diseases or personalised medicines, said Ibara, adding: “We have to partner to find that scale.”

Supporting sites

Capturing possible inclusion data is only part of the puzzle, he went on, explaining there was a need to “bridge the gap between data and humans”.

“Our goal is placing humans in clinical trials, so just capturing the data isn’t enough. We need to convert the sites, but also provide the infrastructure they need,” he said.

Moog said that most healthcare systems wanted to take part in research, but simply did not have the wherewithal to do so.

“There is a lot of interest in clinical trials, but maybe not the administrative staff to handle them, or the training around things like GCP,” he said.

There are often concerns around how to comply with regulatory filings and submissions, or a lack of familiarity with the research environment. Sites often also need to be able to demonstrate a steady stream of clinical trial work to justify the investment.

Collaborative projects, such as the Cerner Learning Health Network, which all three companies are involved, offer the support willing sites and healthcare systems need to take part.

“It is a nationwide network of provider organisations who have agreed to allow access to their data, and who are ready and willing to accept clinical trial, quality improvement, and outcomes research opportunities,” said Moog.

“It really minimises the lift on the parts of all parties, and accelerates their opportunity to participate, even if they haven’t historically been a research organisation.”

Ultimately, it’s about making sure the sites that hold the data have everything they need to make use of it, from the right software to a principal investigator.

Closing the feedback loop

Harnessing the power of RWD from EHRs is not a one-way street: the sector also needs to ensure insights from routine care can be used to guide future research.

Stankus said: “Typically, we take a linear view that science drives evidence, and that this evidence leads to care… unfortunately, we see a lot of missed opportunities, waste, and harm throughout the process. The patient experience often doesn’t feed back into science and evidence.

“What we need to move forward is an integrated clinical care programme… where we are continuing to learn and evolve, and to understand the best thing for patients, as well as for clinicians, clinical workflows, and communities.”

Barriers to this approach include a lack of data quality and data completeness in some EHR systems, he told delegates.

“Research studies are designed to understand specific data, they have data dictionaries that are standardised across sites, and they collect all the information for the outcomes.

“EHR, on the other hand, was originally designed for billing; there are time pressures that clinicians are under, there are unstructured formats, and varying data coding between health systems. All this makes it challenging to extract useful information,” he said.

Greater standardisation of data fields and codes, as suggested by the FDA, could help to shift this dial, and ensure insights are not wasted.

“There are many data that aren’t captured in EHR systems that could really enhance their value, whether it’s understanding certain symptoms, severity, or the worsening of control in certain situations.

“Patient reported outcomes, as an overall umbrella, is an area where EHR could really build capacity and understanding on how people, throughout the process, are experiencing their health conditions, not just as a patient, but frankly, as a whole person.”

Aligning the data

It has been a long time coming, but now, with the right technology and outlook, the industry is at a “tipping point” in terms of harnessing RWD to Excellerate patient outcomes, said Stankus.

But the answer does not lie solely in technology – success will depend on working together to build and provide access to the right datasets, as well as ensuring everyone who needs them has the right tools in the right hands.

Oracle co-founder and Chief Technology Officer Larry Ellison made a surprise appearance during Microsoft Inspire 2022 this week to promote a new collaboration between the two tech giants, a collaboration that Microsoft CEO Satya Nadella called “a great opportunity for our partners.”

The companies announced Oracle Database Service for Microsoft Azure, a new, fully managed service for provisioning, accessing and operating enterprise-grade Oracle Database services in Oracle Cloud Infrastructure (OCI) with an Azure-like experience. The collaboration comes after Oracle executives acknowledged trouble moving customers from on-premises environments to the cloud, a more profitable environment for vendors.

Since 2019, the companies have collaborated on OCI-Azure Interconnect for secure, private interconnections with sub-2 millisecond latency in 11 global regions.

“Now, we’re making it even easier, launching this new experience that streamlines and simplifies deployment even further – everything from cross-cloud network setup to identity management,” Nadella said. “Once deployed, customers will be able to launch and manage the Oracle services they use every day from Azure so that they can manage their apps and data as a single solution [and] run their most important apps across our cloud platforms seamlessly.”

The partnership between Oracle and Microsoft speaks to the growth of multi-cloud environments, Ellison said.

“You stay in the Azure console, and you can take that Oracle Autonomous Database and run [Microsoft] Power BI against it for analytics, accessing Microsoft services as if they were in Oracle,” Ellison said.

He continued: “Customers are very excited about this because customers like choices. … Now its easy to migrate from on-premises to the cloud. Now it’s easy to get the best persistent data store with the best analytic tool and use them together, regardless of who the supplier is. Customers like that.”

Customers using the service will have no data egress or ingress charges when moving data between OCI and Azure, according to Oracle. The companies also have a collaborative support model to speed up issue resolution through either company.

Customers avoid complex rearchitecting or replatforming to use services from both companies, Nadella said.

Both platforms widely used already

Thousands of customers already run production workloads across both cloud platforms. Joint customers include AT&T, Marriott International, General Electric and FedEx, according to the companies.

Nadella called the new collaboration “a great opportunity for our partners to help customers run their most important Oracle apps and databases with Azure as they work to speed their cloud migration journeys.

“We have collaborated for decades to help our customers use technologies like Windows Server and Oracle databases together,” he said. “And this partnership really extends that work to the cloud. Bottom line is, we are both very focused on addressing the tough business and technical challenges of our customers. And that's what we’re going to do together.”

The news comes amid great competition between the two tech giants. Microsoft is regarded as having more cloud market share than Oracle, with Microsoft usually ranking just behind Amazon Web Services.

Microsoft and Oracle busy in M&A

Both have made major acquisitions in the health care technology space. In June, Oracle closed on its US$28 billion purchase of health care information system provider Cerner. In March, Microsoft closed on its $16 billion purchase of voice-to-text software provider Nuance, which has a large presence in health care.

The outspoken Ellison has also taken swipes at Microsoft in the past, including declaring Oracle’s cloud offering “much better” than the Big Three of AWS, Microsoft and Google.

“That’s been the big question out there -- was our cloud good enough to compete with Amazon and Google and Microsoft,” Ellison said on a quarterly earnings call in September. “And I think we have answered those questions – it’s not only good enough to compete, in many cases, it’s much better. For security, for performance, for reliability, for cost -- we’re cheaper.”

In a December quarterly earnings call, Oracle CEO Safra Catz said that a Gartner report put Oracle’s cloud offering “higher than where Microsoft – who has been in this longer than us – was a year ago.”

On that same call, Ellison did pay Microsoft a compliment, saying that the company has been able to convert on-premises customers to cloud customers more quickly than Oracle.

“A lot of companies like Microsoft did a great job of moving their entire Microsoft Office install base into the cloud to dramatically increase the size of their cloud business,” Ellison said. “Unfortunately, we didnt have the same option or opportunity.”
